1.Acceptance of These Terms
PostAcuteTools (the “Site”) is operated by Josh Camerer / PostAcuteTools.com (“we,” “us,” or the “operator”). By accessing or using the Site or any of its calculators, simulators, reference pages, downloads, or other tools (collectively, the “Tools”), you agree to be bound by these Terms of Use & Disclaimer (these “Terms”). If you do not agree, do not access or use the Site or the Tools.
2.What the Tools Are — and Are Not
The Tools are intended to help you explore and understand Medicare payment methodologies and related billing and clinical reference information. They produce estimates and illustrations only. Certain values used by the Tools (including, where noted, case-mix weights, base rates, per-visit amounts, and thresholds) may be sample, placeholder, prior-period, or simplified values and may not reflect current, final, or facility-specific figures.
- The Tools are not an official payment determination, pricing decision, or coverage determination by CMS, a Medicare Administrative Contractor (MAC), or any payer.
- The Tools are not medical, clinical, legal, accounting, tax, financial, coding, billing, or other professional advice, and no professional or advisory relationship is created by your use of them.
- The Tools do not substitute for the official CMS final rules, the CMS Pricer software, your MAC’s guidance, a qualified coder or clinician, or your own independent professional judgment.
3.Data, Privacy & Email Updates
This site is designed to store zero data. There is no database, and nothing you enter into a calculator is saved, logged, or transmitted — every tool runs entirely in your browser. When you close the tab, your inputs are gone.
The only personal information we collect is your email address, and only if you voluntarily subscribe to our update list. We use it for a single purpose: to notify you when a new tool is added or when CMS rates are updated. We do not sell, rent, or share it.
Email subscriptions are handled by our email provider, Kit (kit.com), acting as a third-party processor; your address is stored in their system and is subject to their privacy policy. We use confirmed (double) opt-in, and every email includes an unsubscribe link you may use at any time.
Because the site stores no data and operates no database, no patient, claim, or other protected health information (PHI) you work with can be retained or exposed through this site. Avoiding storage entirely is how we eliminate PHI risk.
4.No Warranty
THE SITE AND THE TOOLS ARE PROVIDED “AS IS” AND “AS AVAILABLE,” WITHOUT WARRANTIES OF ANY KIND, WHETHER EXPRESS, IMPLIED, OR STATUTORY. TO THE FULLEST EXTENT PERMITTED BY LAW, WE DISCLAIM ALL WARRANTIES, INCLUDING ANY IMPLIED WARRANTIES OF ACCURACY, COMPLETENESS, TIMELINESS, RELIABILITY, MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, AND NON-INFRINGEMENT.
We do not warrant that the Tools, their calculations, or any reference content are accurate, complete, current, or error-free, or that the Site will be uninterrupted or secure. Medicare rates, rules, codes, and methodologies change frequently; results may be outdated or incorrect at any time. You are responsible for independently verifying any result against the applicable CMS final rule, the relevant Pricer, current code sets, and your MAC before relying on it.
5.Your Responsibility
You are solely responsible for how you use the Tools and any decisions you make based on them. You agree not to rely on any output of the Tools for actual claim submission, billing, clinical, contractual, investment, or compliance purposes without independent verification by a qualified professional and confirmation against authoritative sources.
6.Limitation of Liability
TO THE FULLEST EXTENT PERMITTED BY LAW, IN NO EVENT WILL THE OPERATOR BE LIABLE FOR ANY DIRECT, INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, EXEMPLARY, OR PUNITIVE DAMAGES, OR FOR ANY LOSS OF PROFITS, REVENUE, DATA, REIMBURSEMENT, OR GOODWILL, ARISING OUT OF OR RELATING TO YOUR ACCESS TO OR USE OF (OR INABILITY TO USE) THE SITE OR THE TOOLS, WHETHER BASED IN CONTRACT, TORT, NEGLIGENCE, STRICT LIABILITY, OR ANY OTHER THEORY, EVEN IF ADVISED OF THE POSSIBILITY OF SUCH DAMAGES.
Because the Tools are provided free of charge, to the extent any liability cannot be fully disclaimed, the operator’s total aggregate liability arising out of or relating to the Site or the Tools will not exceed US $50.00 (or the amount you paid to use the Tools, whichever is greater). Some jurisdictions do not allow certain limitations, so some of the above may not apply to you.
7.Indemnification & Hold Harmless
You agree to defend, indemnify, and hold harmless the operator and its affiliates from and against any and all claims, demands, liabilities, damages, losses, costs, and expenses (including reasonable attorneys’ fees) arising out of or related to your use or misuse of the Site or the Tools, your reliance on any output, or your violation of these Terms or any applicable law or third-party right.
8.Third-Party Links & Services
The Site may link to or rely on third-party websites and services (for example, CMS resources). We do not control and are not responsible for the content, accuracy, availability, or practices of any third party. Your use of any third-party service is governed by that party’s own terms and policies.
9.Intellectual Property
Except for third-party and government content, the Site’s design, text, and original tools are owned by the operator. You may use the Tools for your own internal, non-commercial reference purposes. You may not copy, scrape, resell, or republish substantial portions of the Site without permission.
Third-party code sets and standards. The Site references standardized healthcare billing and coding systems owned or maintained by their respective organizations. Code numbers are used as factual references, and any descriptions are paraphrased or summarized for educational purposes — they are not official text. Always verify against the official source before relying on them. In particular:
- CPT® (Current Procedural Terminology) is copyrighted and is a registered trademark of the American Medical Association (AMA). Any CPT codes shown are illustrative examples only; no CPT code descriptions are published here, and this Site is not certified by, affiliated with, or endorsed by the AMA.
- The UB-04 / CMS-1450 form and its data specifications — including condition, occurrence, value, revenue, type-of-bill, and patient-status codes — are maintained and copyrighted by the National Uniform Billing Committee (NUBC) and the American Hospital Association (AHA). Descriptions here are paraphrased; consult the official NUBC UB-04 Data Specifications Manual.
- X12 EDI transaction standards (such as the 837 and 835) and their implementation guides are copyrighted by ASC X12. Loop and segment identifiers are referenced as facts; implementation-guide text is not reproduced.

These Terms are governed by the laws of the State of Missouri, USA, without regard to its conflict-of-laws rules. You agree that any dispute arising out of or relating to the Site or these Terms will be brought exclusively in the state or federal courts located in Greene County, Missouri, and you consent to their jurisdiction.
