CY2026 KX modifier threshold: $2,480 for PT & SLP combined and $2,480 for OT. PT and SLP amounts pool into one bucket; OT stands alone. Append KX to claims that go above the threshold — the claim that merely reaches it doesn't need it yet. The $3,000 targeted medical-review line holds through 2028.
